Your First Visit

Thank you for your interest in our office!

We are pleased to welcome you to our practice and to have the opportunity to assist you in the maintenance of your oral health.  You will find that we will work diligently to always deserve your confidence. 

Your initial visit will be concerned with the identification of your personal dental health needs and desires.   All emergency complaints and painful situations will be handled immediately.  Our intention is to provide superior service and solutions to the reasons for your consultation with us.

Our initial exam appointment  will take give us the opportunity to become well acquainted.  During your visit we will review your medical history, take any necessary x-rays,  perform an oral cancer screening, evaluate the foundation and gum tissues supporting your teeth, check for cavities and broken or worn out restorations, and  evaluate the function of your jaw joints and related muscles.  Recent technology such as Magnified digital photographs of your teeth,  Laser Cavity Detector,  Doppler Jaw Joint Evaluator,  Electronic Pulp Testor,  Transilluminator to detect cracks and fractures,  and Digital Microscopic analysis  may be utilized as needed (at no additional cost!) 

We want to know how you feel about your mouth and smile.  All our findings will be fully explained and then the many options for treatment will be presented in as much detail as you desire. 

Availability

Our office is open Monday, Tuesday, Thursday with occasional Saturdays and Wednesdays with hours that vary from 7:45 am to 7:00pm.    We will work to accommodate your schedule with evening and Saturday appointments.  We’re available for emergency care and will usually see you or your family on a same day basis.

Registration -  forms and questionnaires

The confidential information requested by the enclosed questionnaires and forms will help us serve you at your initial visit.  You may complete and submit this form online or print it out and bring the completed forms to your initial visit.  Please also bring any recent x-rays, nightguards or retainers, and models to our diagnostic visit.

If you have dental insurance, please bring a copy of your ID card and any information you may have to help expedite your claim.  We are always glad to help clarify any questions you might have regarding your coverage.  Payment is expected at the time of service.  Should you choose to pay with a charge card, we honor Visa, MasterCard, American Express, Discover, and have CareCredit and Capital One payment plans available.
